Commercial HVAC Maintenance Agreement

This agreement is entered into between J Maness Company ("Contractor") and the business identified below ("Customer"). By submitting this form, the Customer agrees to the terms of the annual commercial HVAC maintenance plan as outlined. All plans are priced per unit — each additional unit receives a 10% discount.

4. What's Included

Each maintenance visit includes the following services:

Inspect and clean evaporator and condenser coils
Check and tighten all electrical connections
Lubricate all moving parts
Check refrigerant levels and inspect for leaks
Inspect and replace air filter (customer-supplied)
Test thermostat calibration and operation
Inspect ductwork for leaks or damage
Check and clear condensate drain line
Inspect heat exchanger (heating season)
Measure and record system performance data

6. Terms & Conditions

Please read the following terms before signing.

This agreement covers preventive maintenance only. Repairs, parts, and refrigerant are billed separately at standard rates.

The Customer agrees to provide reasonable access to all HVAC equipment on the scheduled service date.

J Maness Company will contact the Customer at least 48 hours in advance to schedule each visit.

This agreement is valid for one (1) year from the date of the first scheduled service visit.

Either party may cancel this agreement with 30 days written notice. Unused visits will be refunded on a pro-rated basis.

J Maness Company is not liable for pre-existing conditions discovered during maintenance visits.

All payments are due upon receipt unless otherwise stated in the terms section of the invoice.
